Therese King and Frank Galarza are our Surgery Admissions Coordinators. You can contact them at 212-659-6821, Fax # 212-426-2050, if you have any questions concerning your admission to the Mount Sinai Medical Center.

Therese and Frank will work with you to schedule your pre-surgical testing as well as arrange pre-certification for your admission and surgery with your insurance carrier. They will also provide you with your hospital admission date.  You will need to fax them your dental clearance prior to your pretesting/admission date.

Patients are responsible for all referrals. Preauthorization is not a guarentee of payment, it is just a notification of your admission.

Cardiac Pretesting and Day of Surgical Admission are located in The Guggenheim Pavilion, 1190 Fifth Avenue on 101st Street, 5th Floor (5 West), Cardiac Registration Desk.
